It's the mouth and eyes I reckon, Arnulf - I suspect it's harder to get them and their lighting right than you'd think. When RKS poses Milly with her eyes and mouth shut or almost so, she can look amazingly sexy otherwise she can appear rather hard and aggressive, or very startled and manic.

I don't know whether there's a lighting trick or similar that can soften the impact of open eyes and mouth, but it could make a major difference - the way Rastifan handled the eyes of the previous image makes me wonder whether he could give RKS a hint or two
