The reply to this has been given several times. If you're not sure then hit the "H" key and the arrow shows you where to PLACE her hand (don't move it, if you are on the right spot it will move by itself).

However look out for the last position as there seems to be a bug when it sometimes points to a spot below the waistband of his jeans - and I've yet to find out how she can reach that.

After many attempts I found that you can actually play with BOTH nipples... The perspective is all wrong so it doesn't look like it would be possible, but it is.

My overall impression is that it's a wonderful game, great consideration has been given to the story and graphics. loved the doctor part.
however I felt that the gameplay requires too much effort from me, which took fun away in some parts. I felt it especially in the scene at the AV room with the blonde tit job.
